      Just copy the code and put it where you want this box to render. A similar looking subscription box is available as wordpress plugin (sorry, I don’t remember its exact name).

      Sorry that code is not written by me! A blog who published this wasn’t live last time when I checked it! BTW A wordpress plugin has similar looks and working but I don’t remember it’s URL.
